Google Forms

I created a form that is basically a way for me to get to know my students.  I titled it Welcome Survey as it will be done within the first few days of starting class.  This will allow me to get a basis as to the background information on my students should I need to send home letters, etc.  Forms like this could be completed by students during class time, emailed to parents, friends, or coworkers, it could be filled out at parent teacher conferences, PTA meetings, or faculty events.   Follow either form to view my form or spreadsheet.
